Student waiver & parental consent agreement

for participation at F’Aerial Arts Circus School

F’Aerial Arts cares deeply about the wellbeing of its enrolled students and endeavours to provide a safe,  fun and supportive environment to learn circus arts.  

1. I recognise that F’Aerial Arts provides circus training for the benefit of myself/my child and that  participation in activities at F’Aerial Arts are a conscious choice. I understand that, to the best  of their ability, F’Aerial Arts staff will provide a safe and trust worthy environment to train in circus skills, but they cannot keep an eye on every student at all times.   

2. I’m aware that circus training does involve a balance between safety and danger. While F’Aerial Arts holds health and safety paramount, I must assume and accept all responsibility for any personal injuries. I must also acknowledge that in the highly unlikely and extreme case of paralysis or  death that F’Aerial Arts cannot be held responsible, and I hereby waive and release F’Aerial Arts and its premises at The Little River Community Hall, Little River, and Rolleston Community Centre, Rolleston from any liability for injuries or  other harm and damages resulting from my/my child’s participation on the premises.   

3. I am aware that circus training is fun but can also be very challenging, and that there are certain risks that cannot be eliminated such as: exposure to bruises, burns, and overall discomfort. I  recognise there are potentially more serious risks, including sprains, fractures, paralysis, broken bones and psychological distress.   

4. As a student at F’Aerial Arts I am responsible for myself and my fellow students and accept that the  use of alcohol or drugs before/during classes is completely unacceptable, and that I am to  report to staff if there is any suspicious activity in this regard.   

5. Circus performance and instruction in circus activities requires that students are touched by the  instructors to demonstrate correct postures, correct form, technique, or to prevent an unsafe  act. I understand that when in engaged in circus activities the participant will also be touched  by other students or staff. The student agrees to be touched where at all appropriate in regards to circus activities and respectful conduct. The participant is expected to report to the staff with  any concerns in this regard.   

6. Finally, by completing this enrolment form  the participant/legal guardian grants F’Aerial Arts the right to photograph and/or video record you/your child’s image, silhouette and any other  reproductions of physical likeness and voice recording in and with the exhibition, theatrically,  on television or online internet in which they may be used or incorporated into advertising,  marketing or publicising of F’Aerial Arts or Circus Arts.
